Sit & Stay 2023 Current Paperwork

Please download and fill out the paperwork with up to date information and please scan along with a copy of your pets vaccination records and email back to the following address: Katie.SitandStay@gmail.com

Contracts on file  with Sit & Stay are good  for the duration of three years.  After three years, a new contract must be filled out and information must be updated.

 For all Sit & Stay Clientele,  vaccine records must be submitted anually to keep on file.

All Sit & Stay Clientele must have a a credit card on file, and updated paperwork by April 1st, 2023 in order to continue services.
